First off, congrats; You made it through some seriously life changing and life shaking content and are making decisions to elevate your life from it. Not an easy path. But we stand on it with you and we see a vision for a world that makes all of the work, worth it.

We are so grateful for the consistent constructive feedback throughout this course, and now we would like to hear back from you in an official capacity.

We want your feedback and testimonials on the course content, your experience navigating it and how you feel it has transformed you.

We have two forms. You can fill out both, just one or none - but we’d be grateful and would greatly benefit from your time on these.

Feedback

This will be private and used to continue to tweak and elevate the content for you and your lifetime access to it.

Testimonial

This will potentially be shared publicly. We will request your consent to share it publicly and give you options to be named or anonymous. We will capture your answers from the feedback form to specific questions, offer the opportunity for you to form them into a testimonial or allow us to, and then request your final approval and consent before using any edited testimonials.

Authorization and Release Information

I understand my testimonial as outlined above (the "Testimonial") and made on behalf of Three Six Nine Media’s On Notice course and may be used in connection with publicizing and promoting On Notice. I authorize Three Six Nine Media to use my name, and the Testimonial as defined on this form.

I hereby authorize Three Six Nine Media to copy, exhibit, publish or distribute the Testimonial for purposes of publicizing Three Six Nine Media On Notice course or for any other lawful purpose. These statements may be used in printed publications, multimedia presentations, on websites or in any other distribution media. I agree that I will make no monetary or other claim against Three Six Nine Media for the use of the testimonial statement.

I understand that I have the right to revoke this authorization to Three Six Nine Media at any time. I understand if I revoke this authorization, I must do so in writing and present my written revocation to Three Six Nine Media. I understand that the revocation will not apply to information that has already been disclosed.

I hereby hold harmless and release Three Six Nine Media from all claims, demands and causes of action which I, my heirs, representatives, executors, administrators or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.
